By this I give my consent to the Private Higher Educational Institution “Ukrainian Humanitarian Institute”, hereinafter – the High School “UGI”, located at the address: 08292, Ukraine, Kiev region, Bucha. St. Institutskaya, 14, for the processing of my personal data specified in the application and / or the questionnaire filled out on the site http://ugi.edu.ua/. The processing of my personal data can be carried out by any action or set of actions, such as collecting, registering, storing, storing, adapting, modifying, renewing, using and distributing (distributing, implementing, transmitting), depersonalizing, destroying personal data, including using information (automated) systems and / or access to such personal data, as well as the transfer of this personal data to authorized representatives of В UGI ’. Processing can be carried out with the aim of implementing educational programs of the В UGI ’foundation and also receiving advertising messages about the services of реклам UGI’.
By this I confirm that the personal data indicated in the application and / or application form completed on this site are correct. The consent is given for a period while there is a need and can be withdrawn by me at any time by written notice to the ChVUZ “UGI”, at the address: 08292, Ukraine, Kiev region, Bucha. St. Institutskaya, 14.
I am warned that sending me the said written notice of revocation of consent to the processing of personal data may entail the termination of my participation in educational programs of PEI “OIG”.
If the application and / or the questionnaire filled out on this site indicates the personal data of the minor whose legal representative (parent, adoptive parent, guardian) I am, then I give the consent of ВIHI В to the processing of these data in ways and conditions above.
